You must fill in this worksheet detailing your contact information, preferred listing format, and fees:

You must also fill in the appropriate application for each of the disciplines under which you want to be listed:

Submit the completed worksheet, Historical Resource Consultants List Application(s), and remittance to:

The current fee for listing is $150.00 per person per year. Renewals for subsequent years are due April 1 to avoid inactivation. The renewal form can be found here.
